Osteoporosis Canada FLS Registry Submission

Osteoporosis Canada (OC) invites Canadian FLSs to submit their FLS model to the new OC FLS Registry. The Registry will be an online map profiling FLSs in operation across Canada, which meet OC’s Essential Elements of Fracture Liaison Services. The Registry will allow insight into the breadth and location of FLS across the country. It will serve as a vital tool to assist with OC’s efforts to foster the implementation of effective FLS across Canada.

Why Submit

The OC FLS Registry will benefit existing FLS programs by providing a resource to showcase your program to stakeholders in your community. It will also be a means for you to discover other programs across the country.

Submit Your FLS Program Today!

  1. Download the submission form. Save the form to your computer and then fill it out. Do not fill out the form online. Persons with an older version of Adobe Reader (i.e. older than Adobe Reader XI) may encounter difficulties saving the form and are encouraged to download a newer version, which is available for free online. This easy to fill out form should take only approximately 20-30 minutes to complete. It will allow us to find out more about your model and determine if it demonstrates OC’s Essential Elements of Fracture Liaison Services
  2. After you have filled out the submission form, save it and send your completed submission to FLSRegistry@osteoporosis.ca
  3. All submissions will be de-identified prior to being sent to the FLS Registry Committee
  4. FLS models demonstrating OC’s essential elements of Fracture Liaison Services will be profiled on the OC FLS Registry map, which will be available for viewing in the coming months. This will include an icon showing the location of the FLS and a brief description of the model. There will be no rating system and you will have an opportunity to consent to the description of your program before it is placed online.

Submissions will be reviewed on a rolling basis and the FLS Registry will be updated regularly throughout the year.
